Complete an entry for each entity in your immediate supply chain, at a minimum those entities identified on your audit trail documentation. THIS DOES NOT INCLUDE INGREDIENT SUPPLIERS. Ingredient suppliers will be listed in H3.3, H3.4, and H3.5. Please upload either a current organic certificate OR Exempt Handler Affidavit in the supporting documentation column for each operation. Exempt Handler Affidavit templates can be found in Certification Docs.
